Registration is open to children 18 months through 5 years of age.
Each child, before the admission day, must have a physical examination by a doctor to be certain that the child is in good physical health and free from any communicable diseases. Also, a complete report of the child's medical history and immunization records must be submitted prior to admission.
Our center is operated on a non-discriminatory basis, providing equal treatment and access to service without regard to race, color, religion, national origin or ancestry.
State law requires that the following forms be completed and returned to the school office on or before the first day of attendance:
- Proof of Immunization
- Identification and Emergency Information
- Child's Pre-Admission Health History - parent's Report
- Physician's Report
- Acknowledgment of Parent's Rights Notification
- Consent for Medical Treatment
- Personal Rights Acknowledgment
- Parent Handbook Policy Acknowledgment and Signed Admission Agreement
No child will be admitted until all the above forms are completed. You may pick up the forms in the preschool office.
An annual, non-refundable registration fee is required of each child. See financial rate sheet for current rate. March is re-registration month and the annual fee becomes payable. For new families registration is in April, call for specific date. |
